Middletown High School is a public high school in Middletown, Ohio. It is the only public high school in the Middletown City School District.

- Boys Basketball – 1944, 1946, 1947, 1952, 1953, 1956, 1957.[2] The Middies' 7 boys basketball state titles are the most of any Ohio high school.

- Butch Carter – Former player and Head Coach in the National Basketball Association, brother of Cris Carter
- Cris Carter – NFL Hall of Fame, Pro-Bowl and, All-Decade player and analyst, brother of Butch Carter. Member of the NFL 1990s All-Decade Team.
- Jeff Cothran – (born June 28, 1971 in Middletown, Ohio) is a former fullback in the NFL. He played his entire three-year career for the Cincinnati Bengals.
- Jerry Lucas – Legendary NBA Hall of Famer, NBA champion, Olympic gold medalist, author, and world-renowned memory education expert. Later named one of the NBA's 50 greatest players
- Clarence Page – Pulitzer Prize winner, journalist, syndicated columnist and member of the editorial board for the Chicago Tribune
- John M. Watson, Sr. – trombonist with Red Saunders and Count Basie Orchestras and other notable musical acts, educator, actor in 13 movies (Bones Roosevelt in The Fugitive, Uncle Pete in Soul Food) and several theater roles including the Broadway production of One Flew Over the Cuckoo's Nest.
