This article contains information on rugby league played in the UK at semi-professional level in 2004. For general information on the National Leagues, see Rugby League National Leagues.

In January 2004, LHF Healthplan agreed to sponsor the National Leagues,[1] and the competition was subsequently renamed as the LHF Healthplan National League. Leagues One and Two were made up of the 18 clubs that competed in the Northern Ford Premiership during the previous season, plus London Skolars and York City Knights. National League Three expanded to a 14-team league, but there was still no automatic promotion and relegation with National League Two.

National League One was won by Leigh Centurions, and won promotion to the Super League after defeating runners-up Whitehaven in the play-off final. Keighley Cougars were relegated to National League Two.

National League Two was won by Barrow Raiders, and were automatically promoted to National League One. York City Knights reached the play-off final, but lost to Halifax, who retained their National League One status.

National League Three was won by Coventry Bears, who also went on to win the play-offs.
